Damaged Passport

DS-11 Passport Application

• Complete and print 2 copies (Single-Sided Only) of the Online Government Passport Application Form DS-11.

• Hand-written applications are NOT accepted.

• Ensure bar codes (DS-11 B) are located at the bottom right hand corner of pages 1 & 2.

Damaged Passport

• The damaged passport must be included within the Sealed Envelope and it will not be returned.

• The damaged passport cannot be used as Proof of Citizenship or Identification.

Statement of Mutilated Passport

• Print out and neatly fill in form with Black Ink Only. (2 Copies)

• The signed and dated original must be included in sealed envelope.

2x2 Passport Photo

• Color photo must be 2 x 2 inches and have a white background and be professionally taken within the past 6 months.

• No uniforms or headgear, as well as, no white shirt.

• Eyeglasses are no longer allowed to be worn in passport photos.

• We recommend visiting a local Rocket-Passports parter to ensure the perfect photo.

Proof of U.S. Citizenship

• Must be an ORIGINAL or CERTIFIED copy with a raised or embossed seal of an U.S. Birth Certificate (with both parents names on it).

• An Original Naturalization Certificate is acceptable.

• A Marriage Certificate or Divorce Decree is required if the applicant's last name has changed.

• An expired Passport can serve as Proof of US Citizenship.

Proof of Identification

• A valid copy of ID that has been issued GREATER than 6 months ago must be included (Front & Back Copy).

• ID can be a Drivers License, State ID, Military ID, or any Gov't ID. Paper, Learner's Permits and temporary licenses are not acceptable.

Travel Itinerary

• Most commonly used itineraries are airline, cruise & hotel reservations.

• A Business Letter on Original Company Letterhead can be used in lieu of travel itinerary.

• The Travel Itinerary and DS-11 application dates must match and include the applicant's name. Print 2 Copies

Government Fees

• A personal or cashiers check payable to "US Department of State" in the amount of $211.36 for the Passport Book only or $241.36 for a Book and Card.

• A pre-printed Name and Address must be located in the upper left hand corner. It cannot be a starter or temporary check.

• Ensure that the applicant's full name and date of birth is written in the memo section of the check or money order.
