2x2 Passport Photo
Color photo must be 2 x 2 inches and have a white background and be professionally taken within the past 6 months.
No uniforms or headgear, as well as, no white shirt.
Eyeglasses are no longer allowed to be worn in passport photos.
We recommend visiting a local Rocket-Passports parter to ensure the perfect photo.
Current Passport
Original Name Change Document
You must send an ORIGINAL or CERTIFIED COPY of your marriage certificate, divorce decree, or court order.
All Original Name Change documents will be returned by government.
Travel Itinerary
Most commonly used itineraries are airline, cruise & hotel reservations.
A Business Letter on Original Company Letterhead can be used in lieu of travel itinerary.
The Travel Itinerary and DS-11 application dates must match and include the applicant's name. Print 2 Copies
Government Fees
A personal or cashiers check payable to "US Department of State" in the amount of $211.36 for the Passport Book only or $241.36 for a Book and Card.
A pre-printed Name and Address must be located in the upper left hand corner. It cannot be a starter or temporary check.
Ensure that the applicant's full name and date of birth is written in the memo section of the check or money order.
Expediting times start once documents are received.
